Does it have to stay on a garage theme?

No. The front panel is one attachment surface and the brim is a second one. Is the surface hook and loop, and how does a patch stay on?

Yes. Hook on the patch meets loop on the hat. The loop side is Patchistry Fiber, a weave made for this hat rather than a stock panel sewn onto the front. It runs the full width of the front panel and continues across the brim, so there are two real surfaces instead of one small square.
